Well Cared For Incorporated (“WCF” or “we”) provides a Software as a Service platform (the “Platform”) and related services (the “Services”) which allow individuals (“End-Users”) to create profiles for themselves and their loved ones, including pets and family members (“Family Members”) to collect and share health information and communicate with joint caregivers and health-care professionals (“Professionals”). The Platform also provides tools for other activities, including scheduling health-care appointments, managing medications, and tracking symptoms.
This privacy policy (“Privacy Policy”) covers WCF’s collection, use, transfer, storage and disclosure of information about identifiable individuals, including their personal health information, through the Platform and Services. Personal health information includes any information about an identifiable individual: (i) concerning their physical or mental health;
(ii) concerning any health service provided to them; (iii) that is collected in the course of providing health services to the individual; or (iv) that is collected incidentally to the provision of health services to the individual (collectively, “Personal Information”).
1. What Information We Collect
1.1 Account Registration
In order to use the Platform, End-Users and Professionals must have a valid Well Cared For account (“Account”). When End-Users and Professionals register for the Platform and Services, WCF collects certain types of Personal Information, depending on the type of user, including first and last names, nickname, date of birth, gender, address, phone number,
profession, business address, business name, email address, and photographs.
1.2 Health Profiles
After an End-User registers an Account, WCF collects the following Personal Information of End-Users and Family Members:

1. What Information We Collect

1.2 Health Profiles
After an End-User registers an Account, WCF collects the following Personal Information of End-Users and Family Members:
- contact and identification information, including first and last name, phone number, address, email address, photograph, and relationship to other Family Members;
- demographic information, including date of birth, gender, height, weight, hair colour and eye colour;
- health information, including symptoms, conditions, scans, charts, medications, family medical history and photographs; and
- health management information, including names and contact information for health care providers, dates, times and locations of appointments, and insurance policy information.
2. How We Use Your Information
2.1 Account Management
WCF uses the Personal Information collected upon Account registration to uniquely identify each End-User and Professional as a user, authenticate their Account access, and provide them with access to the Platform and Services. WCF also uses this Personal Information for record keeping, account recovery, to prevent fraud or future abuse, and data analytics.
2.2 Providing Services
Personal Information is collected from End-Users, Family Members and Professionals and is used to allow the individuals to easily share the Personal Information with others, including Professionals, to provide the Services, in order to facilitate health care.
2.3 Maintaining and Improving Platform
The Platform tracks certain information about usage of the Platform, including IP addresses, time spent on the Platform, frequency of log-ins, etc. (“Usage Data”). WCF uses Usage Data to diagnose technical issues with the Platform, monitor and improve Platform efficiency, monitor and create logs for security purposes, and to inform product development and Platform improvement.
2.4 Advertising
Usage Data and Personal Information may be used for marketing purposes to allow WCF and its partners to provide customized advertising recommendations. If you opt-in to receive such notifications, we can send you information for health care and related services or products which may be of interest to you based on your Personal Information.
3. Where and How We Store Your Information
WCF stores all of the Personal Information it collects in the course of providing the Platform and Services using cloud services on servers located in Canada. WCF currently uses the services of MedStack, Inc. (“MedStack”). MedStack’s data and infrastructure security and privacy management protocols have been designed based on the NIST and ISO27001 standards.
4. How Your Information is Shared
4.1 At your discretion
While using the Platform and Services, you may choose to share certain Personal Information with others. To access the Personal Information, the other person must be registered as either an End-User or Professional. You can control what information you share with other users and what information you keep private from other users.
4.2 With service providers
We may from time to time retain other companies and people to perform tasks on our behalf (“Service Providers”) and need to share Personal Information with them in order to provide the Platform and Services. Such Service Providers do not have the right to use the Personal Information we share with them beyond what is necessary to assist us.
4.3 Change of Control
If WCF, or substantially all of WCF’s assets, are acquired, or if we enter bankruptcy, or go through some other change of control, Personal Information may be made available or otherwise transferred to the new controlling entity, where permitted under applicable law.
4.4 With your consent
We may share your Personal Information for purposes that are not identified in this Privacy Policy if we have your implied or express consent.
4.5 Where required by law
WCF may disclose your Personal Information where required by law, including disclosing to the relevant authorities if compelled by law to do so.

We will keep your Personal Information for as long as it remains appropriate for the identified purpose or as required by law, which may extend beyond the termination of our relationship with you.

There may be some limited circumstances where we are unable to provide you with a full copy of your Personal Information, such as where it relates to another identifiable individual.
